You get a much more accurate measurement if you have someone assist you and take your measurements for you. You should be wearing only underwear.
BUST - Place the measuring tape over the back, and across the fullest part of the bust (with the bra on) keeping the tape measure parallel to the floor. Do not pull the measuring tape too tight, just keep it a little snug when you breath in and fill your lungs with air. Your arms should be parallel to the floor, out to the sides, making your body a T shape.
WAIST - Place the measuring tape around the smallest part of your waist. Note: The true waist measurement is much higher on your body than where most girls wear their skirts or pants.
HIPS - Place the measuring tape around the fullest part of your hips with the tape measure parallel to the floor. This is typically 8" or more below your waist line.
If the dress you choose has a full skirt, then you should choose a size to fit the larger of you bust or waist measurement. The skirt will fit in the hip. If you choose a style with a slim skirt you should choose a size to fit the larger of your 3 measurements.

Most wedding dresses need some minor alterations for a "custom fit." A seamstress can always take in a dress that is too big, but cannot make a dress fit that is too tight.
